A large provider for elderly services in the state of Massachusetts is hiring several RN Case Managers to make an immediate impact.
The Registered Nurse Case Manager is responsible for completing initial and subsequent clinical eligibility assessments for patients placed in nursing facilities within the organization. In addition, the Registered Nurse assesses interested nursing facility residents for readiness to return to community settings under various government programs.
Qualifications:
- Bachelor of Science Degree in Nursing, Associates of Science degree in Nursing or Diploma RN
- Current, Active, Unrestricted license to practice in the state of MA
- 2+ years experience in clinical setting and/or nursing in the community or LTC setting. RN Diploma requires 4 years experience
Schedule: Monday - Friday, 9am-5pm
Pay Rate: Based on experience
