Details:
-Monday thru Friday, 9:00am-5:30pm
-Pay Rate: $22-$24/hour
Requirements:
-Medical assistant or pharmacy background
-Ability to Multi Task-work off of 3 monitors
Main Responsibilities:
-Working with Referral sources to gather discharge information notes/ to educate/setup patient.
-Working within 5 different portals--technical
-High sense of urgency
-Must keep an eye out for new referrals coming in---follow up on existing and new.
-Referrals coming in can be for patients being discharged from Hospital same day or tomorrow.
-Data entry for referral
-No verification of benefits ---only gathering info for coordination of benefit team to process.
-Go through a documentation checklist of what is needed to send to benefits team for next steps.
-Responsible for securing "Nursing Agencies" for additional services needed.
-Must be able to read a patient chart--and navigate through to pull up the information.
-Onboarding a patient-permissions and coordinating signatures.
